Once you have purchased your A Modo Mio machine, follow the six golden rules below and you will receive your cheque for £20 within 28 days of receipt of application form.

THE GOLDEN RULES:

1. This promotion is valid when you purchase an A Modo Mio machine
between 9th March and 7th May 2012.

2. You must send the original barcode, cut from the machine
packaging. See example bar code in the pdf you can download below.

3. You must send a photocopy receipt (showing machine model, amount and date of purchase).**

4. Complete the application form which you can download below

5. Detach the form and post it to: Lavazza A Modo Mio Offer, P.O. BOX 716, High Wycombe, HP11 1XT making sure you enclose the requested proof of purchase.

6. Make sure it reaches us by the 7th June 2012.

**These items will not be returned. Please allow up to 28 days on receipt of voucher for delivery, gift set and cheque may arrive separately.

TERMS AND CONDITIONS: This voucher may only be used by a customer who has purchased a Lavazza A Modo Mio espresso machine in the UK between 9th March and 7th May 2012. Customers who return their vouchers in accordance with the instructions overleaf will be sent a cheque for £20. This promotion is open to UK residents only aged 18 or over, other than Lavazza employees, contractors, their family or friends.
Only one claim per person will be accepted. Proof of posting of your claim will not be accepted as proof of receipt and no responsibility will be accepted for any claims lost, damaged or delayed in transit.
Cheques will be dispatched via first class post to the address provided on the claim form below within 28 days of receipt of this voucher and proof of purchase. There is no cash or credit alternative. Cannot be used in conjunction with any other Lavazza promotion.
If you do not comply with any of these terms and conditions, or your claim is incomplete or incorrect, the promoter may at its complete discretion disqualify your claim. The promoter’s decision will be final.
You will be ineligible to claim your cheque, and any claim you make will be disqualified, if you have cancelled or received a refund for your purchase.
If your claim is disqualified, you must either return the cheque or (if it has already been paid into your bank account) send a cheque for £20 to Lavazza Coffee (UK) Ltd at the address below.
The promoter reserves the right to take legal action if you fail to comply with this provision. By entering this promotion, you agree to be bound by the above terms and conditions and that the promoter may contact you about the purchase of your A Modo Mio.
